1 entry per person • Winner will receive free LASIK surgery. Winner is
required to schedule evaluation appointment to determine whether they
are a candidate for surgery. • Merchant cancellation/re-scheduling
policy of 72 hours applies • If customer is determined to be a
non-candidate for LASIK due to medical reasons found during the
preoperative evaluation, gift can be redeemed for PRK laser vision
correction (if appropriate) or transferred to another person • No cash
